Day Habilitation Specialist I at The Arc of Oneida Lewis Chapter

GENERAL STATEMENT OF DUTIES: Ensures implementation of the person we supports' ISP/Day Habilitation Plan by providing direct care, training, and supervision to the extent required; by utilizing as many community based resources and supports as appropriate to the plan. IMMEDIATE SUPERVISOR: Program Director QUALIFICATIONS: High School Diploma or G.E.D with specialized training or one year experience in working with persons with developmental disabilities; or in an educational setting or in a health care facility. As Habilitation Specialist's will be involved in providing transportation to the people we support, an approved copy of a Driver's Abstract from the Department of Motor Vehicles must be available. Requirement of an approved Driver's Abstract may be waived by the Division Head. Children's Case Manager 2 at Transitional Living Services of Northern New York

Promotes the ability of high risk individuals to achieve independent living skills and stability in their psychiatric or medical conditions. Case/care management links individuals to service systems, then coordinates and monitors the provision of services. Services are individualized to the specific identified needs of each person in a culturally sensitive way. This is achieved using a holistic approach considering several areas of strengths and needs including, but not limited to the individual's: history, culture, spiritual preferences, education and employment information, social involvement, mental health and substance abuse status, medical needs, housing advocacy and needs, safety issues, legal issues and financial issues. The case/care manager encourages and assists the recipient to develop natural community supports and use community resources to encourage stabilization and integration into the community. This position may require use of personal vehicle to transport clients when an agency vehicle is not available. Respite Care Provider at Transitional Living Services of Northern New York

Transitional Living Services seeks overnight care for children with serious emotional disturbances in our Case Management Program. Care Providers will provide overnight child care respite for families on an emergency or planned basis. The goal of this service is to provide the child and family a break to promote an increase in family harmony. Care Providers supervise and engage the child while in their care with an environment and activities that support his/her constructive interests and abilities.
