Job Description:


GENERAL STATEMENT OF DUTIES: Performs public health nursing activities with individuals and families in accordance with the planned program of the agency; does related work as required.

DISTINGUISHING FEATURES OF THE CLASS:       Under supervision performs public health nursing activities involving responsibility for assessment of health needs and developing the plan of care for individuals and families.  The work is distinguished by the responsibility of the incumbent for initial implementation of nursing care plans and providing nursing care.  Supervises Registered Professional Nurses and staff assigned to carry out the plans of care.  The incumbent is also responsible for periodic re-evaluation of individual and family nursing needs.


  • Gives skilled nursing care and prescribed treatments to patients in their homes and clinics and demonstrates nursing care to patients and families;
  • May lead a team of peers and subordinates providing nursing care and evaluates the effectiveness of team activities;
  • May perform duties such as pediatric nurse or family nurse practitioner;
  • May provide for the continuity of patient care by promoting referral of hospital and clinic patients to appropriate community agencies;
  • Coordinates plans for service with nutritionists, social workers, physical therapists, physicians and other professional workers concerned with individual and family health care;
  • Counsels and guides individuals and families toward self-help in recognition and solution of physical, emotional and environmental health problems;
  • Provides nursing services in clinics and schools;
  • Teaches classes, addresses groups, and participates in community planning related to nursing and health;
  • Participates in the conduct of surveys and studies related to health matters;
  • Compiles and uses records, reports and statistical information for evaluation and planning of the assigned programs.


Thorough knowledge of current public health nursing practices; working knowledge of the administrative organization of community facilities; skill in the application of current nursing procedures and techniques of patient care; ability to plan and coordinate nursing care for individuals, families and groups and supervise nursing personnel assigned to assist with this care; ability to communicate effectively; ability to establish and maintain cooperative working relationships; ability to accept and utilize guidance; ability to perform duties in accordance with ANA Code for Professional Nurse; acceptable physical and mental health to carry out the responsibilities of the position.


A bachelor’s degree or higher in nursing from an approved school acceptable to the New York State Department of Health.


Eligibility for a New York State Registered Professional Nurse’s license at time of application.  Possession of the license at the time of appointment.


DISTINGUISHING FEATURES OF THE CLASS:       The work involves responsibility for performing a variety of professional nursing tasks involved in patient care and community health protection.  The work is performed under general supervision with leeway allowed in the exercise of independent judgment in carrying out the details of the work.  Does related work as required.


  • Provides skilled nursing care and prescribed treatments to patients;

  • Visits patients in the home and assesses patient nursing care needs and develops nursing care plans;

  • Identifies physical and emotional illness of patients through observation and analysis of records;

  • Coordinates plans for patient care with nutritionists, social workers, psychologists, physical therapists, physicians and other professional health care workers;

  • Instructs patient and family members in regard to patient’s diet, health habits and of the carrying out of treatment at home and rehabilitation measures;

  • Sets up and staffs prevention and immunization health clinics, including taking patient history, conducting screening physicals involving taking blood pressure, pulse, respiration and weight, drawing blood and administering tests and vaccines for various diseases;

  • Counsels and guides individuals and families in the prevention of illness and the attainment of maximum health;

  • Prepares and maintains a variety of nursing care records and reports;

  • Attends in-service training to keep abreast of latest nursing techniques and procedures;


Good knowledge of modern public health nursing and clinical practices; good knowledge of auxiliary and support services available to provide for the total health care of patients;   application of modern nursing techniques and medical equipment used in providing nursing care; ability to develop and implement a nursing care plan; ability to communicate effectively with other staff and patients and their families; ability to understand and follow oral and written instructions; ability to establish and maintain effective working relations with others; ability to participate in assigned projects and lay-out work for others; ability to organize and maintain accurate records and files; ability to understand and empathize with the needs and concerns of others; good powers of observation; physical condition commensurate with the demands of the position.

MINIMUM QUALIFICATIONS:  Licensure and current registration as a Registered Professional Nurse in New York State.
